Repairs to be completed as per the estimate. If additional parts and/or repairs are required, authorization for these supplementary items will be required, from the payer, before proceeding further.
I hereby authorize the above repair work to be done along with any necessary materials. You and your employees may operate the vehicle for purposes of testing, inspection or delivery at my own risk. An express mechanic’s lien is acknowledged on vehicle to secure the amount of repairs thereto. You will not be held responsible for loss or damage to the vehicle or articles left in the vehicle in case of fire, theft, accident or any other cause beyond your control.
Should I choose to cancel the repairs at any time, I will be responsible for any accrued labour, parts and parts restocking costs.
